My fee is $170 per session and $200 for the initial session/intake.

Insurance

I accept Premera/Lifewise/Anthem/Heritage NOW, United Healthcare/Optum, Aetna, Oscar Health, Oxford, and First Choice Health network (Kaiser PPO, HMO with pre-authorization), as well as some other insurance companies.  For all insurance, please call your insurance provider prior to our first meeting to ask about:

• Your yearly deductible (amount of $$ you are responsible for paying)

• Your copay or co-insurance (a fee each session you need to pay)

It is your responsibility to learn about your particular insurance plan.  The number for your insurance company should be listed on the back of your insurance card.

If you have an insurance provider that I’m not in-network with I can bill out-of-network, and you might be reimbursed for part of the cost. Check with your insurance provider for coverage. I can provide invoices or monthly statements for you to submit to your provider for reimbursement upon request.

Cancellation

If you need to cancel our appointment, please contact me at least 24 hours prior to our session (and the earlier the better!) If you cancel with less than 24 hours notice or do not show up to our session, you will be charged for the full cost of the session (up to $170 fee).
